VHHA, the Virginia Health Care Association (VHCA) and a statewide task force began work in 2007 to develop a user-friendly "universal transfer form" to facilitate information sharing when individuals move between hospitals and nursing facilities. In addition to VHHA and VHCA, the task force included the Virginia Association of Nonprofit Homes for the Aging (VANHA), the Virginia Health Quality Center (VHQC), the Virginia Department of Health and the Virginia Department of Social Services.
After a pilot of the form during the summer and fall of 2008 and revisions recommended following the pilot, the transfer form (PDF) is available for use by hospitals and nursing homes. The model form provides a single page for the nursing facility to enter information useful to the hospital upon transfer of the resident/patient to the hospital and a single page on which the hospital will enter treatment and contact information for use by the nursing facility upon the resident’s/patient’s transfer back to the nursing facility. The form also can be used for transfers to and from other settings, including assisted living facilities. The form can be completed online or can be printed and completed manually.
